For a family facing a parent’s dementia, a service-connected rating can mean monthly compensation, health care priority and caregiver support. VA’s rules give several ways to make the link.
Three ways to service-connect dementia
- Direct: evidence that the condition began in, or was caused by, service1.
- Secondary: a disability caused or worsened by a service-connected condition can itself be service-connected2.
- TBI presumption: for a veteran with a service-connected TBI, VA presumes presenile dementia of the Alzheimer type, frontotemporal dementia and dementia with Lewy bodies are caused by it if they appear within 15 years of a moderate or severe TBI, absent clear evidence otherwise2.
How VA grades the TBI
| Factor | Mild | Moderate | Severe |
|---|---|---|---|
| Loss of consciousness | 0 to 30 min | Over 30 min, under 24 hours | Over 24 hours |
| Post-traumatic amnesia | 0 to 1 day | Over 1 and under 7 days | Over 7 days |
| Glasgow Coma Scale | 13 to 15 | 9 to 12 | 3 to 8 |

Missing the severity or 15-year limits doesn’t bar a claim: VA then decides it under the general rules of service connection2. Parkinsonism after moderate or severe TBI is also presumed2.
Agent Orange and Parkinson's
Parkinson’s disease and parkinsonism are presumptive Agent Orange conditions for veterans with qualifying service3. Dementia isn’t on that list, but if Parkinson’s is service-connected and causes or worsens cognitive decline, the dementia may be claimed as secondary2. See Agent Orange conditions.
How dementia is rated
VA rates neurocognitive disorders under diagnostic code 9326 using the general mental disorders formula4:
| Rating | Key features |
|---|---|
| 100% | Total occupational and social impairment: e.g., disorientation to time or place, intermittent inability to perform daily activities, memory loss for names of close relatives or one's own name |
| 70% | Deficiencies in most areas, e.g., spatial disorientation, neglect of hygiene, inability to adapt |
| 50% | Reduced reliability, e.g., impaired short- and long-term memory, impaired judgment |
| 30% | Occasional decrease in work efficiency, e.g., mild memory loss |

Step by step
- Submit an intent to file to protect the date6.
- File VA Form 21-526EZ, online, by mail, in person or with an accredited representative7.
- Include medical evidence: the diagnosis, the dementia type, and a doctor's opinion on the link to TBI or another service-connected condition8.
- Add lay statements from family about symptoms and changes, on VA Form 21-102108.
- Attend the C&P exam, with a family member who can describe daily functioning7.
Worked example
A Vietnam veteran, 74, was knocked unconscious for two hours in a 1969 truck accident, and VA service-connected his TBI. He was diagnosed with frontotemporal dementia in 1982, within 15 years of a moderate TBI (loss of consciousness over 30 minutes), so VA presumes the dementia was caused by it unless there’s clear evidence otherwise2. If his symptoms now include disorientation and inability to manage hygiene, a 100% rating under 9326 could apply4. If the dementia had begun decades later, he could still claim it with a medical opinion linking it to the TBI2.
Common mistakes and denial reasons
- No TBI severity evidence. Records of loss of consciousness, amnesia or Glasgow Coma Scale scores determine whether the presumption applies2.
- No medical opinion on the link when the presumption doesn't fit2.
- Only describing memory. Ratings turn on overall social and occupational impairment, so describe daily functioning, safety and self-care4.
- Not claiming the underlying condition. Secondary service connection needs the primary condition service-connected first2.
What a rating unlocks
- Caregiver stipend: a combined 70% rating opens PCAFC9.
- Nursing home care: VA must provide needed nursing home care to veterans rated 70% or more10.
- Competency: VA may also decide whether the veteran can manage benefits, which can lead to a fiduciary11.
- Survivors: a service-connected death can qualify a spouse for DIC12.
